CRF INFO

Each chapter must turn in a Quarterly Chapter Report Form (CRF) every 3 months to stay active. They are a summary of the events and initiatives the chapter has held throughout each quarter.

The form asks for items such as (but not limited to):

  • Event descriptions

  • Number of Attendees

  • Funds raised for any fundraisers

Due dates are:

  • January 15

  • April 15

  • July 15

  • October 15

Submission is through a Google Form, which can be accessed here.

Please email me a screenshot of the form submission confirmation page with a date + time stamp.

CUF INFO

Each chapter must turn in a Chapter Update Form (CUF) every year to stay active. They give a chapter active status during the administrative year June 1st- May 31st.

I recommend turning them in as soon as possible so if there's ever an issue with documentation on the national level (which can happen) we can vouch for you.

The CUF is due May 31st to National Headquarters ([email protected] & [email protected]).

Please cc the following people:

The CUF needs the signature of your chapter advisor. Please arrange for this as soon as possible, especially with these current social distancing guidelines.

Your Chapter only needs the school seal if you are starting a new chapter or if you are turning in a chapter update form for the first time in 3 years.

The CUF is not to be confused with the quarterly CRF that is due on the 15th of January, April, July, and October.

QFS INFO

Each chapter must turn in a Quarterly Financial Statement (QFS) every 3 months to stay active.

The form asks for a summary of financial transactions, funds raised, and the chapter budget, for the past three months.

Due dates are:

  • January 15

  • April 15

  • July 15

  • October 15

Submission is through a Google Form, which can be accessed here.

Please email the Regional Treasurer a screenshot of the form submission confirmation page with a date + time stamp.
